How do I go about receiving a copy of the EH&S Certification form?
Click on Forms to locate the Certification in Word or PDF format.
Is support documentation required when submitting the EH&S Certification?
Yes. All work orders or invoices containing completion of repair dates for EH&S deficiencies or a report can be submitted to your assigned HUD Project Manager. The report or work orders/invoices must accurately identify the repairs that have been made to correct the EH&S items. This includes location of those repairs and the date(s) the repairs were made.
How do I obtain a physical copy of the Physical Inspection report?

How do I obtain Access to Secure Systems?
To apply for authorization, please visit the Real Estate Assessment Center (REAC) website: http://www.hud.gov/reac. Once reaching this website, click on the words "Online Registration" located in the Helpful Tools section. This link will direct you to a web page where you can start the registration process. This web page also includes useful information regarding tips on registering and answers Frequently Asked Questions.
How do I appeal my REAC Inspection Score?
There are two types of appeals; Technical Reviews and Database Adjustments. Appeal requests must be submitted to HUD in writing within 30 days for a Technical Review and 45 days for a Database Adjustment. A copy of HUD guidance for submitting appeals titled "Steps for Submitting a Technical Review" which became effective May 1, 2010 can be found at http://www.hud.gov/offices/reac/products/pass/PDFs/guidelines-adj.pdf. Note: This document addresses both Technical Reviews and Database Adjustments.
